Registration/Enrollment/Open Enrollment Process: 
To enroll a student in Hayward Unified School District, please contact Ms. Todd-Frazier at rtodd@husd.us or (510)723-3180 ext. 62144

Infinite Campus/Parent Portal Process:

For enrolled students to upload necessary documents and contact information for the '20-'21 school year:

ENROLLMENT POLICY HAYWARD UNIFIED SCHOOL DISTRICT

MT.EDEN HIGH SCHOOL

IN ORDER TO REGISTER AT MT EDEN HIGH SCHOOL THE STUDENT MUST LIVE IN THE MT. EDEN HIGH SCHOOL AREA WITH HIS/HER PARENT OR LEGAL GUARDIAN AND MUST BRING:

  • PROOF OF UP-TO-DATE IMMUNIZATIONS-A baby book, prior school’s records, or doctor’s/medical provider’s immunization report is accepted as proof.
  • TRANSCRIPT/REPORT CARD/AND WITHDRAWAL GRADES- An unofficial transcript or report card must be provided.  Please be sure the student has checked out of the former school and that grades to the date of leaving are provided.  Failure to provide this information may necessitate a student starting in a lower grade level or being placed in incorrect classes.
  • 2 PROOFS OF PARENT/LEGAL GUARDIAN RESIDENCE– These proofs should be in the form of utility bills in the parent/legal guardian’s name such as P.G. & E, Water, Garbage, Phone, cable, etc.  (We CANNOT accept a driver’s license or personal letters as proof of residence.) You MUST have TWO bills/receipts from utilities to prove residence.
  • LEGAL GUARDIANSHIP PAPERS– If the student is living with a legal guardian a copy of the court order or similar papers must be shown at the time of registration..

PARA PODER REGISTRARSE EN LA ESCUELA MT. EDEN, EL ESTUDIANTE

DEBE VIVIREN EL AREA DE LA ESCUELA CON SUS PADRES O GUARDIANES LEGALES, Y DEBE PRESENTAR LOS SIGUIENTES DOCUMENTOS:

 

  • PRUEBA RECIENTE DE VACUNAS– Puede ser el cuaderno de vacunas de cuando era bebe, pruebas de vacunas presentadas anteriormente a otras escuelas, o pruebas de vacunas extendidas por un doctor o clinica medica.
  • REPORTEDECALIFACACIONES/REPORTE ESCOLAR/ Y ULTIMAS CALIFICACIONES– Un reporte de calificaciones no oficial, o un reporte escolar debe ser presentado a esta escuela.  Por favor asegurese de que el estudiante ya no está registrado en la escuela donde estuvo asistiendo anteriormente y que también haya obtenido todas sus calificaciones correspondientes hasta el último dia de clases en dicha escuela para asi poder presentarlas a Mt. Eden.  Si esta información no es presentada, el alumno tendra que ser reprobado o ser colocado en clases incorrectas.
  • 2 COMPROBANTES DE DOMICILIO DE LOS PADRES O GUARDIANES/TUTORES– La residencia puede ser comprobada presentando tres recibos de servicios electricos, o de agua, o de telefono, o de basura.  (No son aceptadas ni licensias de manejar ni otro tipo de identificación como pruebas de domicilio.)  Es necesario tener dos compobantes de domicilio.
  • DOCUMENTOS LEGALES DE GUARDIANES/TUTORES– Si el estudiante está viviendo con personas guardianes/tutores, debe presentar una orden de la corte u otro documento similar, al momento de matricular.

Alumni Transcript

Alumni records are kept at Mt. Eden for 5 years. Please use the link below to request your transcript. If it has been longer than five (5) years, you must request your transcript from the HUSD Student Records Department (information in transcript request link).
